Eligibility & enrollment
The criteria below match the CBAS section on the Center Programs page. Our bilingual staff guide you and your family through the process; final eligibility and authorization are determined by your Medi-Cal managed care plan or the applicable review process.
Who can join?
- Adults age 18 and older.
- Applicable Medi-Cal eligibility.
- Meets the applicable CBAS program and medical-necessity criteria.
- Eligibility and authorization are determined through the appropriate Medi-Cal managed care plan or review process. Being 65+, being low-income, or simply having Medi-Cal does not automatically qualify a person.
Three steps to enroll

Call us or visit the center — we will walk you through the services in Chinese or English.

Our staff and your family doctor together evaluate health status and care needs.
- 3
Once your Medi-Cal managed care plan or the applicable review process completes the eligibility determination and authorization, participation can begin; coverage follows your plan's rules.
What to bring
- Medi-Cal card and any other insurance cards
- Photo identification
- A current list of medications
- Your family doctor's contact information
