Center Programs
SunnyDay offers three separate programs: CBAS (Community-Based Adult Services), ECM (Enhanced Care Management) and CalAIM Community Supports. They are distinct services with different criteria, referral and authorization processes, and participation is determined by your Medi-Cal plan.
CBAS — Community-Based Adult Services
CBAS is a comprehensive community-based day health program that supports eligible older adults and adults with disabilities who live at home, helping them remain safely and with dignity in their own community.
What CBAS includes
- Professional nursing care and health monitoring
- Physical, occupational and speech therapy
- Mental health services
- Therapeutic activities
- Social services and case management
- Personal care assistance
- Meals and nutritional counseling
- Transportation to and from the center
Who may qualify?
- Adults aged 18 and older.
- Applicable Medi-Cal eligibility together with established medical necessity.
- Eligibility is determined through the managed care plan or designated review process, and depends on health condition, functional needs and coverage.

ECM (Enhanced Care Management) is a care-coordination service under CalAIM. It is a separate program from CBAS and from Community Supports. SunnyDay's ECM services are offered to Health Net Medi-Cal members; eligibility depends on program criteria, referral or authorization, and availability.
How can ECM help you?
- Helps address complex health and life challenges such as lack of housing support, chronic illness and mental health needs.
- A care manager and care team coordinate connections to health and community resources suited to the member's needs.
- Rest assured — your current Medi-Cal benefits will not be affected, you can continue seeing your regular doctor, and you may opt out of ECM at any time.
Food | Nutrition and food support
Help connecting to nutrition and food resources such as meal programs and community food support. What is available depends on your health plan, referral and approval.
Medical | You're not alone at the doctor's office
The care team may help coordinate appointments, follow-up care, language support and health services according to the member's care plan. Services depend on program criteria, approval and availability.
Housing | Worry-free and comfortable living
Depending on individual needs and available resources, the team may connect members with housing applications, form assistance or applicable home-safety resources.
Transportation | Rides & benefits assistance all handled
According to the member's care plan and available resources, the team may help connect members with medical transportation and applicable social services.
What is Enhanced Care Management (ECM)?
ECM is a free service from Medi-Cal for people who have many health or life needs. It helps you get the care and support you need — at the right time, in the right place, from the right people. It is not just about doctor visits or hospital stays; it also helps with other things in life so you can feel healthier overall.
Does it cost money to apply for ECM?
There is no cost to eligible members. Whether you are eligible is determined by your Medi-Cal managed care plan.
What if I don't have a Medi-Cal health plan?

Community Supports
CalAIM Community Supports address the non-medical, health-related needs that affect a person's ability to live safely and independently — care that reaches beyond the clinic.

About eligibility
Not every support is available to every member. What you can receive depends on your Medi-Cal managed care plan, your individual needs, service requirements, medical appropriateness and any authorization or referral. Our bilingual staff will help you check each one.
